While computers have eliminated some types of human errors and replaced humans for some relatively straightforward tasks, they have also created new types of human error, some of which are even more difficult to control than the ones that were eliminated. At the same time, completely replacing human operators for aircraft and even spacecraft is unlikely in the near future. In this informal talk, I will discuss the basis for these two conclusions and what they imply for the way that complex human-computer controlled systems are designed.
