Understanding the Root Cases of Aviation Accidents
Chris Johnson, University of Glasgow, Scotland
This presentation will describe continuing joint work between the Glasgow Accident Analysis Group and NASA Langley to identify the underlying causes of aviation accidents. In particular, it will identify the over emphasis of human error and the under emphasis of organisational issues that has arguably undermined attempts to reduce the incidence of adverse events in aviation.
